About the Metabolic Health Clinic

Thank you for your interest in the Department of Nutrition Sciences’ Nutrition and Metabolic Health Clinic. In our clinic, we provide nutrition therapy to optimize metabolic heath based on your individual metabolic phenotype, which we determine by analyzing your insulin, glucose, and lipid levels. We specialize in nutrition plans for chronic metabolic conditions including:

  • Obesity
  • Type 2 Diabetes
  • Prediabetes
  • Non-alcoholic Fatty Liver Disease

Before we schedule your appointment, we would like you to complete and return this intake form concerning your current health, diet, and health goals. This information will help us formulate the best diet for you.

You may register for one of three levels of treatment: Bronze, Silver, or Platinum:

Bronze Level ($300)

  • Initial visit (1 hour, in-person) with fasted blood draw
  • 4-week follow-up visit (30 min, in-person or virtual)
  • 8-week follow-up visit (30 min, in-person) with fasted blood draw

Silver Level ($500)

  • Initial visit (2 hour, in-person) with fasted oral glucose tolerance test*
  • 4-week follow-up visit (30 min, in-person or virtual)
  • 8-week follow-up visit (30 min, in-person or virtual)
  • 12-week follow-up visit (30 min, in-person) with fasted blood draw

Platinum Level ($700)

  • Initial visit (2 hour, in-person) with fasted oral glucose tolerance test*
  • 4-week follow-up visit (30 min, in-person or virtual)
  • 8-week follow-up visit (30 min, in-person or virtual)
  • 12-week follow-up visit (2 hour, in-person) with fasted oral glucose tolerance test*
  • Health coaching (see info) provided between clinic visits to assist with implementing your diet plan and attaining your personal health goals

*An oral glucose tolerance test tells us how well your body can metabolize glucose and carbohydrates, and how much insulin you secrete when eating carbohydrates. This information can help guide diet strategy. The test also can be an indicator of diabetes risk. Information provided by this test can help us tailor your diet to reduce your personal risk of diabetes, and tell us whether the diet is improving your metabolic health.

Your First Visit

We request that you bring a personal check in the selected amount to your first clinic visit. This fee will cover:

  • Your initial assessment visit with a Registered Dietitian
  • Personalized diet plan with menus and recipes
  • Body composition assessment by bioimpedance analysis
  • Blood draws and laboratory analyses (glucose, insulin, lipids)
  • Follow-up visits according to your selected plan

Your first visit will involve fasting blood work, so please do not eat for 10 hours before arriving, and do not drink any beverages that contain calories (plain water, black coffee, and clear tea are fine). We will measure your fasted levels of glucose, insulin, and lipids.

On your first visit, you will meet with a Registered Dietitian Nutritionist who will discuss diet options with you based on your health and weight goals, and will provide you with materials for implementing your diet. The next visit(s) will be a check-in to see how you are doing, and to answer further questions you may have regarding your diet changes. Your final in-person visit will also involve fasted blood work. At this visit, we can discuss progress and treatment satisfaction, and outline your needs for future visits or follow-up. Additional 30-minute visits are available on an as-needed basis for $60 (consult with a dietitian, no blood work) or $100 (consult with a dietitian, with blood work).

Depending upon your needs, we can also offer a more rigorous body composition assessment and resting energy expenditure assessment for additional fees.
