Partners


The HIV Care Connect program partners across the Southeastern United States are implementing community driven, evidence-based programs that address health disparities to improve the delivery of HIV care.

  1. Build partnerships between the health care sector and other sectors to address the social determinants of health – the social and environmental factors that affect health.
  2. Collaborate with diverse local organizations to connect people with HIV to resources in their communities that provide access to HIV care, medically tailored food, social services, employment training, and transportation.
