
The HIV Care Connect program partners across the Southeastern United States are implementing community driven, evidence-based programs that address health disparities to improve the delivery of HIV care.
OBJECTIVES
- Build partnerships between the health care sector and other sectors to address the social determinants of health – the social and environmental factors that affect health.
- Collaborate with diverse local organizations to connect people with HIV to resources in their communities that provide access to HIV care, medically tailored food, social services, employment training, and transportation.
