CLIA waiver/ ICL License assistance is available to all private schools and school districts regardless of whether it elects to participate in on-site COVID-19 testing services. This program service assists private schools and school districts with the process, paperwork and fees associated with obtaining a CLIA Waiver/ ICL License from the Alabama Department of Public Health to enable them to conduct their own on-site COVID-19 testing of asymptomatic and symptomatic students and staff.
To apply for a CLIA Waiver/ ICL License, the private school or school district will need an MD or PhD to stand as Lab Director, a standing order from a physician to perform the tests, and information about the rapid tests that will be used.
