Project Description:

The goal of this investigation is to characterize risk factors for relapsed refractory disease in patients treated for multiple myeloma following high dose chemotherapy or autologous stem cell transplant. This investigation will be ancillary to the Integrative Molecular And Genetic Epidemiology (IMAGE) Study of myeloma. CaRES Fellows will be working with the existing IMAGE Study team and infrastructure to identify and recruit new participants, provide informed consent, conduct data management, quality control as well as obtain longitudinal data on existing participants. Preliminary analyses may result in publication.

Title of Project: TLR4 polymorphisms and predisposition for skin cancer development
Project Description:

Toll-like receptors (TLRs) play a key role in the innate immune system. Toll-like receptor-4 (TLR4) in particular, appears to play a role in susceptibility to cancer. Of 44 identified SNPs (small nucleotide polymorphisms) in TLR4, the most common is an A–G substitution at nucleotide position +896, downstream of the cDNA start codon, a missense mutation which leads to an amino acid substitution Asp299Gly in the third exon of the TLR4 gene. Pre-clinical studies from our laboratory have shown an association of TLR4 with ultraviolet radiation induced skin cancer. Hence, in this study we will assess the pattern of TLR4 polymorphisms and susceptibility to skin cancer.

Title of Project: 12ML – Addressing Financial Hardship Among Cancer Patients
Project Description:

Financial hardship affects 30-70% cancer patients and can be associated with psychological distress, health-related coping behaviors (i.e., poor adherence to treatment), and a negative impact on patients’ material conditions (i.e., losing income, debt, bankruptcy). At UAB, it is estimated that 50% of cancer patients experience financial distress based on surveys conducted among our gynecologic and breast cancer patients. Currently, limited data exists on best practices to help patients navigate their costs of care.

UAB O’Neal Comprehensive Cancer Center is accredited by the Committee on Cancer by the American College of Surgeons. Through this goal, we are focusing on addressing financial hardship as a barrier to care for our patients. This CaRES project will focus on several research and clinical practice change activities that we hope will improve financial navigation services to better serve our cancer patients. The CaRES student will gain experience conducting research as well as implementing programs throughout a health system. Our goal is for this project to result in more than one manuscript.

We will focus on 1) developing materials for staff training with staff input, 2) performing a literature review on the existing evidence on interventions to address financial hardship for a research manuscript and to inform financial navigation services here at UAB, 3) review existing patient-facing resources and perform qualitative studies with new cancer patients to learn more about how they interface with our health system in dealing with their costs of care, and 4) analyze financial hardship screening data that is already being implemented in GYN ONC and will be deployed across other disease sites.

Participants will return surveys via mail using a provided pre-addressed and stamped return envelope.

Project Description:

The proposed project addresses an understudied and much needed area of cancer research. Few intervention studies have been attempted among metastatic cancer survivors (MCS). Moreover, none have explored the use of vegetable gardening. Previous research has demonstrated the benefits of this approach among survivors of early-stage cancers. The goal of this project is to gather data, through interviews among MCS (n=15), Health Care Providers (n=15), and Master Gardeners (n=15), to inform the development of a community gardening intervention adapted for those living with metastatic cancer.

Analyze Qualitative Data: Qualitative analysis will be conducted via inductive thematic analysis using NVivo software following the step-by-step approach: 1) organizing and preparing data for analysis; 2) reading and getting familiar with the data; 3) coding the data; 4) synthesizing the codes to develop themes; 5) and interpreting the meaning of the themes. Weekly data review meetings will be conducted with the study team to examine the data analysis process, provide critical feedback on emerging codes, and reach consensus on principal themes to be used to inform the proposed Harvest for Hope intervention.

Title of Project: 08DP – Assessing the built environment for physical activity participation in rural Alabama Black Belt counties
Project Description:

Regular physical activity (PA) is linked to lower risk for several cancers (breast, colon, endometrial). Yet, most Americans are inactive, particularly in the rural Deep South where cancer incidence/mortality is also generally higher. Built environment factors such as lack of physical activity facilities etc likely contribute to these existing cancer disparities but comprehensive evaluation is needed. Thus for the current study we will complete Rural Active Living Assessments (RALA) in 6-7 rural Alabama Black Belt counties. The three RALA components include Street Segment assessments to evaluate factors like walkability, safety features, and terrain of individual, specific street segments. The Town-Wide assessment examines community characteristics like population, total area, and the presence of recreation activities. The Program and Policy assessment identifies community programs and policies that support physical activity. These tools have been successfully used in similar, past studies conducted by our research team in the Deep South. Our goal is to provide an updated assessment of built environment for physical activity in these rural counties, compare/contrast with past findings, and use this data to propose environment/policy level intervention strategies.

Project Description:

AMPLIFI is a large project to test different web-based programs to improve healthy eating and exercise in adults with an-obesity related cancer. The AMPLIFI study collects data on prescription and over the counter drugs, and health care use (hospitalizations, ER visits, doctor visits) self-reported by participants. The intern will work on standardizing the drug data for analysis, and summarizing the information collected to advise the team on how to streamline the data collection. The intern will also analyze the drug and health care use data, obtain the available price for each drug and health care visit, and calculate the total drug and health care cost for each participant.

Title of Project: 06MP – AMPLIFI Recruitment and Retention Core – Recruitment experiments and results
Project Description:

AMPLIFI is a large project to test different web-based programs to improve healthy eating and exercise in adults with an-obesity related cancer. The AMPLIFI Recruitment and Retention Core is in the process of recruiting 652 cancer survivors in Alabama, Mississippi, North Carolina and Tennessee. The recruitment process starts with a letter sent to the potential participant’s address, and then a call from the recruiter. In this project, the CARES intern will participate in two aspects of recruitment: 1) Mailing process: the intern will plan and conduct an experiment to address the question of whether sending stamped or bulk mail makes a difference in whether survivors receive the recruitment letter before the recruiter approaches them by phone; 2) Recruitment call process: the intern will be trained in the process of recruitment calls and tracking outcomes. The intern will analyze and summarize the results of both aspects of recruitment, and contribute to assemble the manual of operations for AMPLIFI recruitment. This project is a great opportunity to experience both the process and the challenges of recruitment which is a fundamental component of human subject research.

Telephone-supported interventions have shown success in increasing PA, do not require clinic visits, literacy, or costly technology, and thus may represent a promising strategy for promoting PA for cancer risk reduction in the Deep South, especially when automated with Interactive Voice Response (IVR) systems for improved cost-effectiveness and reach. Thus, we adapted an existing IVR system, used in past HIV studies, for PA promotion and cancer prevention in the Deep South through extensive literature review and formative research [11 focus groups on PA intervention needs/preferences with Deep South Network For Cancer Control community health advisors (CHAs) and community members]. A pilot trial of the resulting Deep south IVR-supported Active Lifestyle (DIAL) intervention (R03CA177538) found high retention and participant satisfaction at 12 weeks. Moreover, DIAL produced larger increases in MVPA (42.5 more min/week) from baseline to 12 weeks than a waitlist control, along with significantly greater improvements in PA self-regulation and social support. Results have informed intervention enhancements in preparation for scale up and dissemination in rural counties.

The current study will involve a fully powered randomized controlled trial of the refined, multi-level 12 month DIAL intervention with a waitlist control condition (N=240 rural, mostly African American adults). Recruitment will be led by Deep South Network for Cancer Control CHAs in Black Belt counties (involved in project since initial formative research). Primary aims include examining arm differences in changes in moderate intensity or greater aerobic physical activity from baseline to 6 and 12 months. Exploratory aims include examining intervention effects on physical performance and psychosocial variables; changes in physical activity from 12-18 months; intervention costs; potential mediators and moderators of treatment efficacy; and potential barriers/facilitators to widespread implementation of DIAL intervention in rural Black belt counties by Deep South Network for Cancer Control.
